Blossom Spas LLC has put in place preventative measures to reduce the spread of COVID-19; however, Blossom Spas LLC cannot guarantee that you will not become infected with COVID-19.

By booking for Blossom Spas Services on Blossomspas.Setmore.Com, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by attending the Blossom Spas LLC Location (at 600 University Blvd, Ste F, Harrisonburg, VA 22801) and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at Blossom Spas LLC may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, Blossom Spas LLC employees, agents and/or Sub-Contractors, other customers and their families. You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to yourself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you may experience or incur in connection with your Blossom Spas LLC Services (“Claims”).

By booking services on Blossomspas.Setmore.Com on behalf of another Customer, you certify that you are permitted to book on their behalf. You understand that the risk of the Customer becoming exposed to or infected by COVID-19 at Blossom Spas LLC may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, Blossom Spas LLC employees, agents and/or Sub-Contractors, other customers and their families. You acknowledge, voluntarily assume the risk and accept sole responsibility of the for any injury to them (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you may experience or incur in connection with their Blossom Spas LLC Services (“Claims”).

By booking services on Blossomspas.Setmore.Com you hereby release, covenant not to sue, discharge, and hold harmless Blossom Spas LLC, its employees, agents and/or Sub-Contractors, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that booking services on Blossomspas.Setmore.com, your release includes any Claims based on the actions, omissions, or negligence of Blossom Spas LLC’s , its employees, agents and/or Sub-Contractors, and representatives, whether a COVID-19 infection occurs before, during, or after receiving services at Blossom Spas LLC.
